modify and enter into the form.

a copy of the form which you can send right back to hostmaster@internic.net
(or reply to their message) and within 24-48 hours you should be sent an e-mail asking you to approve the transfer. Respond to this e-mail and enter "Y" where asked and your domain will be transferred.
